Kingdom Church Strategy
Application Form

Welcome to the Kingdom Church Strategy Partnership Page.

To begin the application process for your church, please follow thse steps.

1. Print out a KCS Info Paccket and give to the governing body of your church. (deacons, church council, elders, board, staff, etc...) make sure they have read all the qualifications of the strategy and are in agreement. Please contact the SLBA with questions or any clarifications needed before applying.

2. Once your church pastor and governing body agree to become a Kingdom Church Partner, Print out the KCS Church Partnership Agreement and have it signed and dated by the Pastor (or duly authorized acting pastor). You may mail the Church Partnership Agreement to: Salt Lake Baptist Assn., 12401 South 450 East, G2, Draper UT 84020; or fax the agreement to: (801) 619-0575. (Be sure to keep a copy for your church records.)

3. Fill out the KIngdom Church Stragegy application form below and submit. (After you submit, print out the confirmation page for your records.)

4. Your pastor, or other designated contact person, will be notified upon being accepted into the Kingdom Church Strategy. Due to limitted funds, churches will be treated on a first-come first- served basis with priority given to those churches who have attended an SLBA Kingdom Church Strategy Orientation meeting. (See scheduled KCS Orientation meetings)
